Footprint to Success: Mental Health Community Support
The Clubhouse model is based on the idea that people living with mental illness deserve to be part of a community that cares. They have skills and talents to share even if they do not know what those talents are yet.
As a Clubhouse member, you will get the support to meet your goals. You will also be encouraged to help other members meet their goals.
Clubhouse members are also eligible for affordable housing — just another way we help our community members get all of the resources needed to live their best life.
Access Community Support for Mental Health
Footprint to Success is a voluntary program, and it is you who decides to attend. As a Clubhouse member, you can select the days and times you would like to attend as well as the relationships and opportunities you would like to develop.
All planning and performing of work at the Clubhouse are done together with the participation of both members and staff. As a Clubhouse member, you are a critical part of the community with shared ownership and shared responsibility for the Clubhouse's success.

Whether or not you already have a support system, you can never have too much. At Footprint to Success, we provide a listening ear and kind shoulder when you need them. When you have great support, you feel accepted, cared for, and safe, which helps you have a brighter outlook on life.

Every member fills a unique role in our community. Regardless of your interests, we are sure that you will find a work unit (or two) to match. Footprint to Success gives our members a sense of purpose as they serve others. Having a purpose helps give meaning to life.
Membership Requirements
To be eligible for membership, an applicant must:
Be interested in attending Footprint to Success Clubhouse — membership is voluntary
Have a primary presenting problem associated with severe and persistent mental illness
Be able to get to Footprint to Success Clubhouse
Not pose a threat to our community
Be at least 18 years of age
How to Apply for Community Support
A. Schedule a Tour of the Footprint to Success Clubhouse
We invite you to come for a tour of the Clubhouse to get a better idea of our work and to help you feel comfortable with our program. Call us at 954-657-8010 or complete our contact form to schedule the tour.
Please print and fill out the below Referral Form and bring it with you to your tour. The box on the second page needs to be filled out by your case manager, psychiatrist, therapist, or doctor.
B. After a Tour, Set Up an Appointment for a Membership
Once you have decided that the Clubhouse is right for you, set up an appointment to start the membership process. Call us at 954-657-8010 or complete our contact form to get started.
